A
social worker is assigned to each resident to provide support and guidance
to the resident and their family members from admission through discharge.
The social worker serves as a liaison to other departments to ensure continuity
of care, including referrals for home care, assistance with community resources
and aides in addressing resident and family concerns.
For residents returning home, the social worker coordinates the numerous
elements in the discharge plan ensuring a safe return to the community. |
