Lutheran Augustana Center for Extended Care and Rehabilitation proudly offers a wide range of services including long term home health care services. The Long Term Home Health Care Program (LTHHCP) is a comprehensive home based program providing a variety of skilled services to meet an individual’s medical, social and environmental needs in the comfort of their home. Each client receiving care and services by the LTHHCP will have an individually prepared plan of care developed by a Registered Nurse. The provision of care is then coordinated by one of our nurses, who make frequent home visits. All care is collaborated with the client’s primary care physician.

A few of the available services offered to eligible clients are: Respiratory Therapy, Nutrition, Social Work and Social Day Care.
