 The Lutheran Augustana Center for Extended Care & Rehabilitation has a rich tradition providing service to the communities of Brooklyn. Formerly known as the Augustana Lutheran Home for the Aged, Inc., the Home began providing care to the infirm elderly in 1908 in a nineteen-room house through the efforts of the Bethlehem Lutheran Church located on Atlantic Avenue. Our present 240 bed state-of-the-art facility located on the campus of Lutheran Medical Center opened its doors in May 1995 and due to the growing needs of the community, we again expanded our restorative rehabilitative services and other therapeutic spaces through new construction in 2002. Lutheran Augustana Center for Extended Care & Rehabilitation today provides twenty-four hour a day skilled nursing care, medical and other professional services for individuals requiring long-term care. The Home also provides short-term nursing/medical sub-acute and restorative rehabilitative care in physical, occupational and speech therapies for those who will be returning to their homes. The Home is a member of Lutheran HealthCare which comprises Lutheran Medical Center, a 476 bed community hospital, the Lutheran Family Health Centers that offer primary care at 41 community clinics, Health Plus, a provider of a Medicaid Managed Care, and related community agencies serving the culturally diverse neighborhoods of Brooklyn. Augustana’s commitment to excellence is premised upon the breadth of knowledge, creativity, initiative and motivation of its workforce. Our positive resident care outcomes are a direct reflection of the compassion and dedication Augustana’s staff have for their work and the extraordinary work ethic displayed in daily performance.
